Every page is scanned for the machine-printed collection-tube codes and downloaded as a CSV with one row per code (last 3 digits only — that's what identifies the tube), in top-to-bottom order. S.No, Name, UTP No and Age are handwritten, so they're deliberately left blank — OCR on handwriting produces convincing-looking wrong answers, not useful ones. Open the CSV in Excel next to the scanned PDF, fill those columns in, set Blood Group and Gender, then upload the completed file below.
