Every page is scanned for printed collection-tube codes (e.g. K39530258 — machine-printed, so OCR reads them reliably)
and downloaded as a CSV with one row per code, in top-to-bottom order. Name, UTP No, Age and Gender are handwritten, so
they're left blank — open the CSV in Excel next to the scanned PDF and fill them in (sheet format is
UTP/Age,
e.g. "418/23" = UTP 418, Age 23), and set Blood Group if known. Then upload the completed file below.
